Loss of Two (2) or More Passports Within a Period of Two Years 

As per Memorandum Circular No. 30-96 dated 18 October 1996 “a passport may not be issued to someone who has declared two (2) lost passports consecutively within a two year period.”  The case should be referred to the Research and Investigation Group (RIG) of the Department of Foreign Affairs for further investigation.  “In cases where three (3) or more passports were declared lost consecutively within a two year period, no passport shall be issued until the applicant is investigated and cleared by the National Bureau of Investigation (NBI)” Pending the resolution of said cases no replacement passport can be issued to applicant concerned.

Travel Documents

1.          Travel Document Application for Children Born in Taiwan

Requirements

a. Duly accomplished travel document application form
b. Duly authenticated (Taiwan District Court and MOFA) Certificate of Birth (4 copies)
c. Duly accomplished Report of Birth - FA Form No. 40 (4 copies)
d. Photocopy of Passport of Filipino parents (3 copies)
e. If applicable photocopy of Marriage Contract/Certificate of parents (3 copies)
f. Passport size picture of applicant (3 copies) taken in the last six months, white background, with collar or closed neck top and not wearing eyeglasses or hat
g. Affidavit of Parent for late registration if applicable

Processing Period

Since regular processing period for authentication of documents is 3 working days then regular processing period for Travel Documents of children born in Taiwan is also 3 working days.

The Head-Services to National in cases where it is for the best interest of all concerned that applicant is repatriated to the Philippines as soon as possible may recommend to the Resident Representative or Head-Consular Services that Travel Document be issued within the day application was filed without paying expedite fees.

2.    Travel Document Application for a Filipino Who Lost His/Her Passport  

Requirements 

a. Duly accomplished Travel Document Application form
b. Police report regarding lost passport
c. Affidavit of applicant on the fact of loss
d. Affidavit of employer/broker if applicable
e. Passport size photo (3 copies) taken in the last six months, white background, with collar or closed neck top, not wearing eyeglasses or hat.
f. Copy of applicant’s passport
g. Original ARC
h. If illegally staying or overstayed in Taiwan – must be accompanied by the arresting police officer or the police officer in charge of his/her case or properly endorsed by the Assistance to Nationals Services

In the case of those under detention all documents are released on the same day application is filed without paying the expedite fee otherwise the regular processing period of three (3) working days applies.

For those who would want the Travel Document released on the same date expedite fee shall be imposed for the authentication or verification or notarization of accompanying documents but not on the issuance of Travel Document.

2.          Travel Document Application for Filipinos with Expired Passports

Requirements 

a.    Duly accomplished Travel Document Application Form
b.    Expired Philippine Passport and copy of the same
c.    Affidavit of applicant to the fact and reasons why passport not renewed before its expiration
d.    Passport size photo (3 copies) taken in the last six months, white background, with collar or closed neck top, not wearing eyeglasses or hat
