1.    Duly accomplished Crew-List Visa Application Form in triplicate

2.    Crew members/Applicants must be in Taiwan at the time application is filed unless under the exemption stated in the Code

3.    Original Seaman’s Book valid for not less than six months of the crew members

4.    Photocopy of Seaman’s Book of each of the crew members – 3 copies

5.    Vessel’s Registration – (3 copies but original may be required for verification purposes)

6.    Passengers Manifest (3 copies)

7.    For fishing vessels - Affidavit of Master or owner of vessel stating the purpose of the trip to the Philippines and a commitment that its crew will not illegally fish in the Philippine waters or do any illegal act according to the laws of the Philippines  

8.    Payment of Visa Processing Fee 

a.    1-40 members – NT$5,000.00
b.    41-100 members – NT$7,000.00
c.    101-200 members – NT$9,000.00
d.    More than 200 – NT$12,000.00

9.    Payment of expedite fee (if applicable) – NT$500.00 per application

Validity of Visa and Duration of Stay: 

The visa as issued is valid for three (3) months from date of issuance and the maximum duration of stay upon entry is 59 days.

Schedule of Release of Visas

If you submit your application form with complete requirements on or before 11.30 a.m., your visa will be ready for release after 5.00 p.m. of the same day.

If you submit your visa application form with complete requirements after 1.00 p.m., your visa will be released after 9.30 a.m. of the next working day.
